Want to make a difference in the lives of others?We want your talents!We're looking for mission-driven professionals to help Livingston Community Health deliver the best quality of care and excellent service to our patients and their families.Summary:The Licensed Clinical Social Worker (LCSW) will provide quality clinical mental health consultation to providers and staff as requested; and provide comprehensive diagnostic mental health assessments, interventions, education, referrals and consultations for patients in the primary care setting. The LCSW will work within our clinic as a member of the primary care provider team and will offer consultation services to primary care providers and patients concerning behavioral health changes to improve patient health status. Our Integrated Behavioral Health (IBH) model's interventions are brief, practical, and effective. The LCSW will also support in the administrative duties and responsibilities to uphold the Policies and Procedures that support the mission of the Integrated Behavioral Health model. The LCSW may work at multiple LCH sites.Essential Functions:1. Assists the primary care team in recognizing, treating and managing behavioral health and psychosocial issues. Collaborates with the medical team to be an integral part of the patient care.2. Conducts patient intakes, focusing on diagnostic and functional evaluations, then makes recommendations to the primary care team concerning the patients' treatment plan objectives.3. Provides consultation and training to the primary care providers to enhance their skills and effectiveness in treating behavioral health concerns.4. Provides brief, focused interventions for patients who need behavioral health services.5. Gives primary care providers timely feedback about the patient's care, treatment recommendations and progress via documentation in the patient's medical record and verbal feedback.6. Screen for appropriate level of care within the mild-to-moderate primary care setting and discuss with patients and medical team about linkage to outside community resources as needed for higher level of care, i.e. County Behavioral Health and Recovery Services (BHRS) for severe mental illness (SMI).7. Assist the medical care team in referrals to psychiatric medication support services for patients either within the tele psychiatry services at LCH or within the patient's managed care plan.8. Initiates follow-up to ascertain how patients are doing and to determine if any changes in treatment plan are indicated.9. Screen all patients 12 years of age and older for possible substance abuse concerns and initiate the appropriate referrals to either internal Substance Use Disorder (SUD) Program or outside community resources as needed. Identify "at risk" patients and develop a treatment plan to prevent worsening of the patient's condition.10. Monitors and coordinates the delivery of behavioral health services for patients as related to patients' health care, including collaborating with other treatment providers, both internal and external resources with the patients' consent.11. Maintains clear documentation within the patient chart to track the diagnosis, patients' response, treatment plan and progress within the LCH policy to complete charting within 72-hours.12. Support the patient in identifying behavioral/ physical health needs to improve chronic health conditions that may be impacting them daily, assist the patient in initiating healthier activities and find resources that promote overall well-being.13. Provides patients with self-management skills and educational information needed so they can be full participants in their own treatment and recovery.14. Reinforce the medical providers' treatment plan to adhere to the medical recommendations, including medication compliance and other behavioral activities recommended.15. Able to offer brief, targeted interventions usually lasting less than 30 minutes.16. Comfortable with short-term counseling, often lasting less than eight visits.Required Education and Experience:1. Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT) or a Clinical Psychologist (Ph.D.), state of CA.2. Bilingual strongly preferred, languages Punjabi, Portuguese and Spanish.3. Current practices and techniques in psychotherapeutic treatment, especially Cognitive Behavioral Therapy (CBT).4. Excellent working knowledge of evidence-based treatments for medical and behavioral health conditions.5. Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions, also known as Warm Hand-Offs from the medical staff.6. Is comfortable with the pace of primary care, working with an interdisciplinary team, and has strong communication skills, written and verbal.7. Has good knowledge of psycho-pharmacology.8. Has the ability to design and implement clinical pathways and protocols for treatment of selected chronic health conditions.9. Flexible and competent with frequent interruptions, constant changes in scheduling and able to manage multiple demands from both staff and patients;
