Palliative experience is highly preferred. Description: The Medical
Social Worker provides biopsychosocial assessments. Crisis
intervention and short-term counseling are key responsibilities.
Advocacy and linkage to resources are essential for patients and
families. Planning for transitions of care is a primary focus. The
role involves providing psycho-education and facilitating support
groups. Consultation on psychosocial aspects of care will be
provided as a member of the interdisciplinary team. The Medical
Social Worker will serve as a liaison to community programs.
Collaboration with the multidisciplinary treatment team on
discharge planning is necessary. Education may be provided to staff
and other related departments. Participation on committees may be
required. Key accountabilities include: Timely gathering of data to
formulate relevant assessments. Establishing and revising the plan
of care based on ongoing psychosocial assessments. Incorporating
developmental stages of patients and families in assessments.
Assessing for discharge planning resources appropriately.
Identifying and addressing suspected situations of abuse and
neglect when necessary. Direct clinical services may include:
Discharge planning and transitions of care. Individual and family
counseling. Crisis intervention and educational support groups.
Linkage to resources as indicated by assessments and care plans.
Establishing therapeutic relationships is crucial. The Medical
Social Worker must recognize diverse cultural norms and spiritual
values. Effective communication to empower patients and families is
vital. Identifying and addressing resistance and conflict during
interactions is expected. Interventions to decrease tension and
assist patients in evaluating behaviors are needed. Self-awareness
and monitoring personal values and emotions are important for
effective intervention. Providing psycho-education related to
health literacy and community resources is part of the role.
Education to interdisciplinary staff on psychosocial impacts is
essential. Linkage to community resources will be coordinated.
Discharge planning and transition of care will include initiating
referrals to appropriate agencies. Advocacy for patients and
families is a key responsibility. Promoting the patient's right to
self-determination is required. Assisting the healthcare team in
understanding patient perspectives is essential. Advocacy for
culturally and linguistically competent care is expected.
Identification of ethical issues may require utilization of the
chain of command. Interventions in suspected abuse and neglect
cases are necessary. Addressing psychosocial barriers impacting
transitions of care is crucial. The Medical Social Worker will work
