Job Description Job Description Full-Time | Outpatient Community Health Position Overview We are seeking a dedicated and patient-centered Nurse Practitioner or Physician Assistant to provide high-quality outpatient primary care within a community health clinic environment. This position offers strong clinical collaboration, excellent provider support, and the opportunity to serve diverse and medically underserved patient populations. Key Responsibilities Provide comprehensive primary care services for patients of all ages Conduct physical exams, health assessments, and diagnostic evaluations Diagnose, treat, and manage acute and chronic conditions Prescribe medications and develop treatment plans within scope of practice Coordinate patient care, referrals, and follow-up services Maintain accurate, timely, and compliant medical documentation Collaborate closely with physicians, nursing staff, and care teams Qualifications Active and unrestricted California NP or PA license Board certification (NP or NCCPA) Current BLS certification and active DEA registration 1–2 years of clinical experience preferred Experience in community health is a plus Bilingual skills preferred Compensation $131,040 – $170,352 annually (based on experience) Benefits Highlights 403(b) retirement plan with employer match Medical, dental, and vision insurance Health Reimbursement Account (HRA) and Flexible Spending Account (FSA) options Generous Paid Time Off and paid holidays Malpractice insurance coverage CME/CEU reimbursement Eligible for student loan repayment programs
